Job Description:
The Care Coordinator Apprentice will support the smooth and efficient running of our domiciliary care service. This role combines administrative responsibilities with occasional field-based duties, giving the apprentice a well-rounded understanding of care coordination, client management, and operational delivery. Working closely with the Registered Manager and wider team, the apprentice will help ensure that clients receive safe, high-quality, person-centred care.
The apprentice will gain hands-on experience in scheduling, communication, compliance, and client assessments while developing strong organisational and leadership skills. This is an excellent opportunity for someone who is confident with computers, has strong communication abilities, and holds a full UK driving licence with access to their own car.
Duties the Care Coordinator Apprentice Will Undertake
Rostering & Scheduling: Assisting with thecreation and maintenance of staff rotas, ensuring all client visits are coveredefficiently and safely.
Client Assessments & Reviews: Supporting seniorstaff during home visits to complete assessments, care plan reviews, and riskevaluations, while also being able to carry out these responsibilitiesindependently when required.
Liaison with Social Workers & External Professionals:Liaising with social workers, district nurses, occupational therapists, andother professionals to ensure coordinated, person‑centred care.
Emergency Field Support: Providing occasional hands‑onsupport to clients during emergencies or staff shortages (full trainingprovided).
Office Administration: Managing documentation,updating digital care records, filing, scanning, and supporting general officeoperations.
Communication & Customer Service: Acting as apoint of contact for clients, families, and care staff, ensuring clear,professional communication always.
Care Planning Support: Assisting with thepreparation, updating, and monitoring of care plans to ensure they reflectclients’ needs accurately.
Compliance & Quality Assurance: Supportingaudits, ensuring documentation is accurate, and helping maintain CQC compliancestandards.
Technology & Systems Use: Using care managementsoftware and Microsoft Office to manage information effectively.
Staff Coordination: Liaising with care workersregarding schedules, changes, supervisions, client updates, and servicerequirements.
Client Relationship Management: Building positiverelationships with clients and families, addressing concerns promptly, andensuring a high standard of service.
Data Entry & Reporting: Recording informationaccurately, preparing basic reports, and supporting management with datacollection.
Team Collaboration: Working closely with theRegistered Manager, Care Supervisor, and wider team to deliver a high‑qualityservice.
General Operational Support: Assisting withrecruitment tasks, training coordination, and other operational activities asrequired.
Essential Requirements
· Full UK driving licence and access to your owncar.
· Good computer literacy, with confidence usingdigital systems and learning new software independently.
· Excellent communication and organisationalabilities.
· Flexibility to support both office and field‑basedtasks when needed.

Qualifications required
BSc, Diploma, or Business Administration in Public Health and Social Care, or another relevant qualification
